POSITION SUMMARY
The Step-Down Navigator is responsible for meeting all eligible patients to discuss the benefits of stepdown services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to eligibility, into Midas for trending report purposes. The navigator will have a thorough understanding of the step-down process and will coordinate between departments. The Step-Down Navigator will be responsible for completing all necessary paperwork for admission with patients.
Qualifications EXPERIENCE/EDUCATION
Graduate of an accredited college in a social work program or related field. A minimum of three years of experience in the care of mental health patients preferred. Must possess CPR (AED) certification and successful completion of CPI training including appropriate use of seclusion and restraint prior to first shift.
SKILLS
Knowledge of external review organization, payor sources, financial planning and contracting (ie. Managed care, Medicare and Medicaid, Tricare) preferred. Must have required 20 hours of continuing education, CPR, First Aid, and CPI as provide by the facility within 90 days of employment.
