ESI is now hiring an RN Clinical Care Coordinator in the Stuart, FL area.

POSITION SUMMARY:

ESI is seeking an RN Clinical Care Coordinator who is responsible for completing initial and ongoing assessments to identify the physical, psychosocial and environmental needs of private pay home health clients. The nurse will develop, initiate and revise the plan of care as needed to ensure quality and continuity of care. Working closely with the Director of Nursing, clinical team and schedulers, the nurse will monitor assigned cases and help to coordinate services. Position includes participation in our after-hours clinical on-call rotation which consists of occasional weekend and holiday coverage. The right candidate will enjoy working amongst a close-knit group of nurses and support staff and will participate in weekly in-office meetings as well as orient new care providers on skills assessments.

QUALIFICATIONS:

  • A bachelor degree in Nursing is required
  • At least one (1) year of Nursing Case Management experience required
  • RN licensed in Florida
  • Clear criminal background check; current CPR
  • Must be able to communicate clearly in oral & written form Should be skillful in organization and time management skills
  • Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff Understanding Position may be stressful and fast paced and requires timely, complete documentation.
  • Involves travel by car, a lot of sitting, standing, stooping and walking as well as the ability to access client’s homes which may not be routinely wheelchair accessible.
  • Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of client’s condition and to perform and demonstrate client care.
  • Must have a valid driver’s license and be willing and able to drive to client’s residences Step Responsibilities Evaluation
  • Ability to walk, stand, stoop, bend, push a cart, or lift during the entire shift
  • Ability to lift up to 25 pounds, including small packages and supplies

DUTIES AND RESPONSIBILITIES:

  • Perform and provide assessments of Client needs and develop Plans of Care in conjunction with Client, family, physician and Agency
  • Performs admission, transfer, reassessment, post hospital visit, resumption of care and discharge paperwork for the client
  • Prepare and present client’s record to the Agency as directed
  • Coordinates client’s services and participates in case conferences, discusses with DON problems concerning the client and how they may best be handled
  • Consults with attending physician concerning alterations of Patient Care Plan; checks with the appropriate supervisor and makes changes as appropriate
  • Submits clinical notes and progress notes and other clinical record forms outlining the services rendered as directed
  • Participate in Agency meetings to collaborate and coordinate ongoing needs and responsibilities. Bring PR or coverage issues to Administrator immediately
  • Perform and provide assessments of Care Provider’s knowledge and abilities. Direct them in performance of their duties
  • Prepare and deliver accurate reports to DON as requested and appropriate
  • Participate in selective conferences and workshops to promote knowledge in community health trends, efficiency in job performance and overall professional growth and promote staff development for same
  • Assist in the evaluation of community needs and plan programs and services to meet identified needs
  • Foster community relations beneficial to the Agency by participating in consultations with physicians, hospitalists, case managers and the like as directed by DON
  • Provide skilled nursing care as outlined in the Nursing Care Plan
  • ON CALL rotation as directed by DON
